- You need a code to start your rebate. If you have a valid code, enter it in the specified field and hit on the “Start My Claim” blue button placed at the bottom of the window.

- On next window, you will be prompted to choose one contact lens brand family from the options provided for which you are interested in receiving rebate information.
- Say, you select “DAILIES Brand Family” option.
- Next, choose the specific contact lens product for which you are interested in receiving rebate information.
- Next, you have to specify have you worn the lenses before or not?
- If you are a new wearer, choose the respective option and fill out the form with required data.
